Adoptee Art Therapy Support Group Intake

If you would like to participate in the group, please complete the intake form. This form only needs to be completed once. Facilitator will protect confidential information and not share it with anyone else. Once this registration form is complete the facilitator will email you the link to sign up for group dates/times. To save your seat you must sign up for the date/time. I look forward to seeing you in the upcoming group sessions.
